Assessment of ocular trauma involves a systematic evaluation to determine the extent of eye injury and guide appropriate management. It begins with a thorough history, including details about the cause of injury (e.g., blunt force, sharp object, or chemical exposure), time of trauma, and any previous eye conditions. Key symptoms such as pain, vision changes, photophobia, or eye discharge are noted.
The physical examination includes inspection for visible damage to the eyelids, conjunctiva, sclera, and cornea, looking for signs like swelling, lacerations, or foreign bodies. Visual acuity is tested for both near and distant vision to assess potential impairment. The pupillary response and ocular movements are evaluated to detect any neurological impact or damage to the eye muscles.
Further assessments involve slit-lamp examination to examine the anterior segment and cornea, checking for abrasions, foreign bodies, or hyphema (blood in the anterior chamber). If necessary, fluorescein staining highlights corneal abrasions. Intraocular pressure (IOP) measurement helps detect secondary glaucoma or internal trauma but should be avoided in cases of suspected globe rupture. Advanced imaging, like CT or MRI, may be used if intraocular foreign bodies or fractures are suspected.
